Job summary

Qualifications

• High school diploma or equivalent; Medical Assistant, CNA or CHW preferred.
• Experience in community health or social services preferred.
• Excellent interpersonal skills with diverse populations, in person and over the phone.
• Interviewing and assessment techniques.
• Ability to motivate patients to participate in the case management program.
• Knowledge of the targeted patient population.
• Valid driver’s license and auto insurance.
• Bilingual English/Spanish preferred.

Responsibilities

• Greet patients and schedule external physician appointments as requested.
• Screen referrals and provide program information and services.
• Follow up on referrals to assess outcomes and provide additional services.
• Document patient demographics and program eligibility accurately.
• Assist patients with insurance enrollment and understanding plans.
• Help patients navigate healthcare and social services, including transportation or interpreters.
