Job summary

Community Health Partnership of Illinois (CHP) in Aurora, Illinois is seeking a compassionate Patient Health Navigator to assist patients in navigating healthcare services and accessing community resources.

In this role you will identify barriers to care, support appointment scheduling and referrals, and coordinate services while maintaining HIPAA compliance and patient confidentiality. Bilingual Spanish preferred to improve access for diverse communities in northern Illinois.

Qualifications

• High school diploma or equivalent is required; additional healthcare education preferred.
• Experience in healthcare, patient navigation, or related social services is preferred.
• Strong communication and organizational abilities; ability to work with diverse populations.

Responsibilities

• Help patients navigate healthcare services and access resources.
• Identify barriers to care and connect to community resources.
• Assist with appointment scheduling, referrals, and care coordination.
• Provide education on available programs and services.
• Coordinate with clinicians and community partners to ensure continuity of care.
• Document patient interactions accurately and timely.
• Maintain patient confidentiality and HIPAA compliance.
