Job summary

Family Health Centers of San Diego is seeking a Patient Engagement Specialist I to recruit patients into our care system and collaborate with communities across San Diego County. The role focuses on connecting underserved populations with our clinics and services.

The position requires at least 1 year of outreach experience; bilingual English/Spanish capabilities are essential. Travel between sites is required; a valid California driver's license and insurance are necessary.

Qualifications

• At least 1 year of experience in sales, patient engagement, community education, or providing outreach programs.
• Associate's Degree strongly preferred or relevant work/life experience may substitute for the degree.
• High School diploma or GED or equivalent required.
• Travel between sites and other locations is required; must have a car and valid CA driver's license and insurance; driving record acceptable.

Responsibilities

• Recruit patients into the program's system of care and develop, implement, and evaluate patient engagement plans.
• Communicate with community members from diverse backgrounds to recruit and engage patients.
• Leverage community resources to connect patients with FHCSD and establish partnerships with community organizations.
• Work collaboratively as a productive member of a multi-disciplinary health team.
• Plan, develop, execute, and evaluate patient engagement initiatives and referrals.
• Perform other duties as assigned.
