• Instruct patients on documents necessary for eligibility and health insurance options.
• Assign patients a PCP.
• Assist patients to choose a Health Plan with Health Care Options.
• Conduct follow-up on applications and renewals.
• Attend trainings and meetings as required.
• Support the organization’s values and goals.
• Perform other duties as directed
