Qualifications

• Familiar with medical, social service and health insurance terminology.
• Ability to type and enter data with a high degree of accuracy.
• Excellent verbal and written skills for communication with patients, clinicians and staff.

Responsibilities

• Coordinate, process, obtain prior authorizations, and request reports for referrals requested by VCC clinicians.
• Collaborate with others to ensure successful tracking and completion of referrals.
• Complete all associated paperwork.
• Organize information from patient records to complete referral authorizations; verify insurance and eligibility as needed.
• Submit referral information including cover letters, notes, imaging or labs as related.
• Provide patients with referral instructions and how to contact the specialist.
• Act as liaison with referral providers and agencies.
• Document referral information in the EHR using NextGen and HVX.
• Use referral tracking system to move referrals through stages and document progress.
• Verify receipt of consults and reports; ensure documents are scanned and forwarded to clinicians.
• Liaise with referral clinicians and agencies; respond to calls, voicemails, and emails promptly.
• Interact with patients, delivering excellent care and resolving referral-related issues.
