Job summary

Urban Ministries of Wake County's Open Door Clinic in Raleigh, NC is seeking a Patient Care Coordinator. This full-time, hybrid role focuses on care continuity, connecting patients to specialty care, managing medical records, tracking lab results, and ensuring no patient falls through the cracks.

You will lead referral workflows, coordinate lab orders, handle HIPAA requests in Athena EHR, support front-office tasks during busy periods, and mentor interns and volunteers.

Qualifications

• High School Diploma or equivalent required.
• Background in healthcare, care coordination, patient navigation, or medical records preferred.
• Experience with Electronic Health Records; Athena experience a plus.
• Strong organizational, HIPAA knowledge, multi-tasking, and communication skills.
• Bilingual English/Spanish abilities preferred.

Responsibilities

• Lead referral workflows for specialty care and assist patients with outside scheduling.
• Primary lead for incoming/outgoing records and HIPAA releases in Athena EHR.
• Track, schedule, and follow up on lab and diagnostic orders.
• Support front-office workflows during high-volume periods and team absences.
• Mentor interns and volunteers and participate in quality improvement projects.
