Job summary

Titanium Healthcare is hiring a Care Coordinator to manage member healthcare maintenance and treatment. You will schedule visits, verify eligibility, and educate patients on resources, coordinating with a care team to ensure effective plans.

Hybrid work includes home-based duties and in-person member visits across Snohomish County. You’ll travel as needed and bilaterally communicate with providers, documenting all care in the EHR system.

Qualifications

• Bachelor’s degree or higher in a related field is required.
• Minimum 1+ years in healthcare, social work, or care coordination.
• Current BLS certification from the American Heart Association.

Responsibilities

• Maintain caseload and conduct monthly in-person visits for members.
• Review and enroll potential members in Health Homes Program.
• Complete referrals and coordinate care plans with providers.
• Educate members and families about treatments and resources.
• Document care in member records using the EHR system.
