Job summary

VNACJ Community Health Center in Red Bank, NJ is hiring a full-time Patient Care Coordinator to guide patients through their care journey by screening referrals, educating about resources, and coordinating community health and social services. Bilingual English/Spanish preferred to serve our diverse patient population.

This role supports the clinical care team, assists with insurance enrollment, and helps patients overcome access barriers.

Qualifications

• High school diploma or equivalent; Medical Assistant, CNA or CHW preferred.
• Experience in community health or health/social services preferred.
• Excellent interpersonal and communication skills with diverse populations.
• Bilingual English/Spanish preferred.
• Valid driver’s license and auto insurance.

Responsibilities

• Screen referrals, educate patients about resources, and coordinate community health services.
• Greet patients, perform initial screening, and schedule external physician appointments.
• Respond to patient requests with information, referrals and follow-up on outcomes.
• Maintain documentation on referrals and patient demographics.
• Assist with health insurance enrollment and understanding plans.
• Assist patients in problem solving barriers related to healthcare, transportation or medications.
• Identify and establish relationships with health and social service providers.
• Maintain relationships with local service organizations.
• Use technology tools to perform job functions and collect data.
• Support RN Care Managers in managing patient health and social needs.
• Coordinate with clinical team for durable medical equipment needs.
• Adhere to professional behavior and organizational policies.
