LifeDOC Health in Memphis, TN is seeking a full-time Patient Care Coordinator to ensure patients receive necessary care and guidance while coordinating with physicians and departments.

You will follow up with patients, document information in the EMR, and educate patients about their healthcare needs, while maintaining HIPAA privacy.

Qualifications

• Bilingual English and Spanish required.
• High School Diploma or equivalent required.
• Call Center experience preferred.
• Healthcare experience preferred.

Responsibilities

• Reach out to and follow up with patients to ensure compliance with medical recommendations.
• Coordinate with departments to address patient concerns and facilitate care.
• Ensure patient information is accurately recorded in the EMR.
• Receive training on EMR, clinical/operational protocols, and patient communication.
• Inform and educate patients about their healthcare needs to improve outcomes.
• Oversee Providers’ daily schedule to support clinical and productivity goals.
• Maintain confidentiality of all PHI in line with HIPAA requirements.
