Job summary

SACRAMENTO LIFE CENTER INC in Sacramento, CA is seeking a Patient Advocate to act as the main point of contact for patients, handling administrative and clinical tasks in person, by phone, or via email.

You will welcome patients, review intake forms, assist with Medi-Cal Presumptive Eligibility, update EMR records, and coordinate referrals. The role requires a friendly, self-directed professional who can multitask in a busy office.

Qualifications

• Must be able to manage phones and patient communication professionally.
• Ability to multitask in a fast-paced clinical setting.
• Self-directed and able to work autonomously with little supervision.
• Personable with a friendly approach to patient interactions.
• Proficiency with EMR/EHR systems preferred.

Responsibilities

• Answers phones and welcomes patients.
• Reviews intake forms to prepare for advocacy sessions.
• Assists clients with Medi-Cal Presumptive Eligibility.
• Administers pregnancy tests according to policy.
• Provides individual and/or couple advocacy; sets plans and goals.
• Updates patient records in EMR and keeps notes current.
• Mails follow up and schedules appointments.
• Directs patient questions to appropriate medical personnel.
