Job summary

SACRAMENTO LIFE CENTER INC in Sacramento, CA is seeking a Patient Advocate to act as the main point of contact for patients, handling administrative and clinical tasks in person, by phone, or via email.

You will welcome patients, review intake forms, assist with Medi-Cal Presumptive Eligibility, update EMR records, and coordinate referrals. The role requires a friendly, self-directed professional who can multitask in a busy office.

Qualifications

  • Must be able to manage phones and patient communication professionally.
  • Ability to multitask in a fast-paced clinical setting.
  • Self-directed and able to work autonomously with little supervision.
  • Personable with a friendly approach to patient interactions.
  • Proficiency with EMR/EHR systems preferred.

Responsibilities

  • Answers phones and welcomes patients.

As a member of the SLC team, the Patient Advocate will handle a variety of tasks including administrative as well as clinical. The Patient Advocate is the main point of contact for any patient. A major part of the day is spent interacting with either in person, on the phone or via email with patients.

Job Responsibilities: Clinic Duties
  • Answers phones and welcomes patients.
  • Reviews completed intake forms to prepare for patient advocacy session.
  • Assist clients with Medi-Cal Presumptive Eligibility.
  • Administers patients’ pregnancy tests according to Medical Policies & Procedures.
  • Provides individual and/or couple patient advocacy. Develops and notates plans and goals for ongoing patient advocacy sessions. Collaborates with male Patient Advocate as appropriate.
  • Evaluates material needs of patients and assures referrals to appropriate community agencies and organizations. Distributes appropriate items.
  • Updates Patient Records in EMR in a timely manner.
  • Updates assigned section of the resource manual monthly.
  • Makes follow up phones calls to patients
  • Makes or changes follow up appointments and divert patient’s medical questions to appropriate medical personnel
Other Required Duties
  • Attends required monthly Staff and Patient Advocate Meetings
  • Completes all required paperwork for employees, signs confidentiality agreement, TB screening, CPR training, Mandatory Sexual Harassment Training, e-training on OSHA, HIPAA, BBP, TB, and Fire Safety.
  • Responsible for continued organization and cleanliness of the Baby Closet.
  • Attends training sessions, “field” trips to community agencies and other meetings as required.
  • Assists at all major events
  • Performs other duties as assigned
Minimum Qualifications
  • Full Agreement with the SLC Mission Statement, policies and rules.
  • Phone management/etiquette skills
  • Ability to multi-task
  • Self-directed and able to work autonomously
  • Personable, ability to welcome patients
  • Computer proficiency (EMR/EHRPreferred)
Desired Qualifications:
  • Certified Medical Assistant
  • Bilingual
Experience
  • Minimum of 2-4 years of previous experience in a clinical setting

Pay Range (based on experience & knowledge) $23-$25 an hour.

Schedule: 24-32 hours a week. Available to work during the following hours
  • Monday - Thursday 9am-4pm
  • Friday 9am to 7pm
  • Occasional Saturday

  • Office Setting with potential frequent interruptions.
