Fresno CBAS is seeking a compassionate Part-Time Intake Coordinator to be the first point of contact for clients in Fresno, CA. The role involves managing intake from inquiry to enrollment, coordinating with care teams, and ensuring accurate documentation while supporting Medi-Cal verification.

The ideal candidate has a high school diploma or GED, experience in intake or case management, and is proficient with Microsoft Office and EHR systems.

Formación

• High school diploma or GED required.
• Associate's or Bachelor's degree in health or human services preferred.
• Prior experience in intake coordination, case management, or healthcare/social services setting.
• Familiarity with Medi-Cal, managed care, or CBAS is a plus.
• Strong organizational skills with attention to detail and accuracy in documentation.
• Excellent interpersonal and communication skills with a client-centered approach.
• Bilingual (English/Spanish) preferred to serve our diverse community.
• Proficiency in Microsoft Office and electronic health record (EHR) systems.

Responsabilidades

• Manage the client intake process from initial inquiry through enrollment and onboarding.
• Conduct intake assessments and gather necessary documentation from clients and families.
• Coordinate with clinical, administrative, and care teams to ensure seamless client transitions.
• Maintain accurate and up-to-date client records in compliance with regulatory requirements.
• Communicate with referral sources, healthcare providers, and community partners.
• Screen clients for program eligibility and assist with Medi-Cal or insurance verification.
• Follow up with prospective clients and families to guide them through the enrollment process.
