Job summary

Fresno CBAS is seeking a compassionate Part-Time Social Worker to join our Fresno, CA team. The role runs Monday through Friday, 8:00 AM - 2:00 PM and focuses on screening, intake, in-home assessments, discharge planning, and care coordination to support CBAS/ADHC participants and their families.

The position includes group sessions and counseling, with advocacy as needed. Ideal candidates hold a BSW or MSW, CA licensure or eligibility, and demonstrate strong communication and crisis

Qualifications

• Bachelor's or Master's in Social Work from an accredited program.
• Active California licensure (ASW, LCSW, or eligibility) preferred.
• Experience in case management, care coordination, or community-based social services.
• Strong knowledge of local Fresno community resources and social service systems.
• Excellent communication, interpersonal, and crisis intervention skills.
• Bilingual in English and Spanish is a plus.
• Compassionate, client-centered approach with strong ethical standards.

Responsibilities

• Screening and intake processes for the Center and follow-up on inquiries.
• In-home assessments for CBAS/ADHC participants and documentation of progress.
• Discharge planning and care coordination for participants and families.
• Develops and documents Person-Centered Plan of Care and IPC services.
• Supervises social worker assistant and/or social work aide.
• Provides group sessions and participant/family counseling.
• Enforces participants' rights and supports grievance procedures.
• Collaborates with community organizations for referrals.
