Job summary

Asian Health Services seeks a PACE Intake Coordinator to ensure older adults and caregivers access full PACE benefits. The role involves intake appointments, documenting medical and social needs, and coordinating enrollment from home, phone, or the Day Center.

You will work with the Outreach team and IDT to deliver a smooth, person-centered enrollment experience, conducting home visits as needed and supporting a coordinated care plan.

Qualifications

• Experience communicating with older adults and caregivers with person-centered, culturally responsive care.
• Strong assessment, triage, and care coordination skills are required.
• Proficiency with documentation systems (EHR/CRM) and strong organizational abilities.
• 2+ years of clinical nursing experience in geriatrics, primary care, home health, SNF, or hospital case management.

Responsibilities

• Receive, triage, and track referrals; ensure completeness and prioritize by acuity.
• Coordinate initial clinical screening and eligibility confirmation for PACE.
• Review clinical records to support eligibility and IDT reviews.
• Coordinate pre-enrollment assessments and home visits as needed.
• Complete level of care assessments and support enrollment transitions.
• Maintain intake documentation per CMS/DHCS requirements.
• Support readiness for audits and external site visits.
• Provide community outreach and maintain referral partner relationships.
