Job summary

Asian Health Services seeks a PACE Home Care Coordinator to join the Interdisciplinary Team. You will perform initial and reassessment home care assessments for PACE participants, coordinating skilled and unskilled care and ensuring equipment needs are met.

You will educate on home safety, collaborate with staff, and maintain documentation in the PACE EHR. Prior nursing licensure and bilingual skills are required for this community-focused role in California.

Qualifications

• RN license in good standing (CA) and BLS certified.
• Bilingual ability (Cantonese and/or Mandarin) with culturally/linguistically appropriate service experience.
• At least 1 year with home or community health provider experience.
• Ability to make home visits and drive to community locations as needed.
• Experience with frail/elderly populations or willingness to receive training.

Responsibilities

• Complete comprehensive initial home care assessments for new PACE participants and reassessments for existing participants.
• Develop and implement home care plans addressing medical, physical, social, and emotional needs.
• Schedule and monitor in-home care delivery with IDT alignment.
• Communicate participant status changes promptly to IDT; monitor health/psychosocial changes.
• Coordinate durable medical equipment and assistive devices for home use.
• Assist with determining qualifications and competency of home care staff and providers.
• Document home care activities in the PACE EHR per regulations and policy.
• Attend IDT and morning meetings and report on participant status changes.
• Support PACE QI programs and participate in QI activities.
