Job summary

Asian Health Services is seeking a PACE Home Care Coordinator to join the interdisciplinary team ensuring culturally competent, home-based care for seniors in the community. You will perform initial and ongoing home care assessments, coordinate durable medical equipment, and supervise in-home services and staff to meet participant care plans.

The role requires an active California RN license, BLS certification, and experience with EHR systems, with emphasis on serving Cantonese/Mandarin-speaking

Qualifications

• Graduate of an accredited School of Nursing.
• Bilingual ability Cantonese and/or Mandarin and experience providing culturally and linguistically appropriate services.
• At least 1 year with home or community health provider experience.
• Ability to make home visits and drive to community locations as needed.
• Experience working with frail or elderly populations (or receive training from PACE).
• Proficiency on a standardized set of core competencies for the PACE Home Care Coordinator.
• Experience working in EHRs (Electronic Health Records).
• Medically clearance for communicable diseases and up-to-date immunizations.
• No disqualifying criminal history related to Medicaid/Medicare, health programs, or fraud.
• Current California RN license in good standing and BLS certified.

Responsibilities

• Participates as an IDT member and completes comprehensive initial home care assessments and reassessments.
• Develops and implements home care plans considering medical, physical, social, and emotional needs.
• Schedules and monitors in-home care delivery with home care staff and contracted providers.
• Communicates status changes promptly to IDT members.
• Monitors health or psychosocial changes requiring reassessment and communicates changes.
• Coordinates delivery and maintenance of durable medical equipment and assistive devices.
• Assists with staff qualifications, competency and performance expectations.
• Documents home care activities in the PACE EHR per regulations and policy.
• Attends IDT and morning meetings and reports on participant status changes.
• Supports PACE QI program and participates in QA activities.
