Job summary

Seen Health is seeking a Nurse Practitioner or Physician Assistant to lead in-home care coordination for the PACE program in the Los Angeles area. You will triage communications, coordinate with PCPs, manage orders and medications, and educate patients and families to support comprehensive, patient-centered care.

The role requires bilingual English/Mandarin proficiency, strong communication and documentation, and the ability to work with a multidisciplinary team to optimize outcomes for seniors

Qualifications

• Current state-issued NP or PA license.
• Bilingual English / Mandarin proficiency required.
• Influential communication skills; capable of care management conversations.
• Strong documentation skills.

Responsibilities

• Triage inbound communications from patients and teammates and prioritize needs.
• Coordinate with PCPs and healthcare professionals; assist with on-site instruction to care staff.
• Oversee post-appointment processes including orders, referrals and medications.
• Prepare comprehensive chart documentation and educate patients on care plans.
• Develop care plans based on nursing process with multidisciplinary input.
• Vaccinate and educate participants about routine vaccines as needed.
• Educate participants and families about chronic disease management and prevention.
• Assess participants’ wellness, needs and preferences to reflect their wishes.
• Perform authorized nursing and case management services in participant homes as needed.
• Support RN and LVN teams with home care and pre-enrollment assessments.
