Job summary

Legacy Community Health is hiring a Medical Case Manager focused on Ryan White services in Houston. The role emphasizes psychosocial support, patient advocacy, and coordination of medical and social services for HIV+ patients.

Qualifications

  • Master's degree in social work is required.
  • Current Texas LMSW is required.
  • One year of case management experience with HIV or related conditions is required.
  • A health/human/education services degree is preferred.
  • Excellent communication and customer service skills required.
  • Knowledge of community resources and HIV treatment issues required.
  • Willingness to travel between locations.

Responsibilities

  • Identify and screen HIV+ individuals to address social service needs.
  • Provide information and referrals for medical and psychosocial linkages.
  • Advocate for clients to ensure access to care and reduce service gaps.
  • Assist clients who have been lost to care to reconnect with services.
  • Maintain documentation to meet program standards.
  • Collaborate with patients to complete assessments and care plans.
  • Update electronic health records within 72 hours of services.
  • Participate in community meetings and training sessions.

Medical Case Manager - Ryan White (Job Overview)
Schedule: Monday-Friday (8AM-5PM)

As the Ryan White Medical Case Manager, you will play a vital role in transforming the lives of our HIV+ patients by providing a supportive and compassionate approach to case management.

Key Responsibilities
  • Identify and screen HIV+ individuals to address their social service needs.
  • Provide information and referrals to ensure seamless medical and psychosocial linkages.
  • Advocate for clients to ensure they have a voice and work to reduce service gaps.
  • Support patients who have been lost to care in reconnecting with essential medical services.
  • Adhere to healthcare standards, including DSHS Texas HIV Management Standards.
  • Collaborate with patients to complete Comprehensive Assessments and Individualized Care Plans.
  • Maintain accurate documentation to meet program standards.
  • Participate in community meetings and training sessions for continuous learning.
  • Monitor service effectiveness alongside peers and supervisors.
  • Update electronic health records within 72 hours of providing services.
  • Contribute to Legacy’s Performance Improvement and Quality Assurance Program.
  • Show respect and compassion towards patients and team members, fostering a supportive professional environment.
  • Work collaboratively to achieve departmental goals.
  • Travel between locations as needed to ensure accessibility for all clients.
  • Embrace a team-based approach in facilitating comprehensive patient care.
  • Take on additional responsibilities as assigned to further our mission.
Minimum Qualifications
  • Master's degree in social work is required.
  • Current Texas Licensed Master of Social Work (LMSW) is required.
  • All licensed professionals must maintain their professional licenses as defined by their respective Texas licensing/examining boards.
  • One year of case management experience with people living with HIV and/or persons with a history of mental illness, homelessness, or chemical dependence is required.
  • A degree in health, human or education services is strongly preferred.
  • Excellent communication skills and customer service skills.
  • Requires a working knowledge of community resources and treatment issues related to HIV.
  • Must be able to manage multiple deadlines and prioritize.
  • Demonstrated competency with an EHRS system.
  • Ability to multitask in a fast-paced environment.
  • Above average organizational skills.
  • Strong typing and computer skills.
  • Must be able to travel between locations as needed.
  • This job requires access to confidential and sensitive information, requiring ongoing discretion and secure information management.
  • Bilingual (English/Spanish) is preferred. If the employee is bilingual, then they must have written and verbal fluency in English and Spanish.
  • Experience working among the gay, lesbian, bisexual and transgender community is preferred.
