Job summary

Legacy Community Health is seeking a Health Navigator to build strong relationships with patients, helping them access essential medical care and stay engaged in their health journey. The role supports medication adherence and collaborates with Legacy Pharmacy partners.

You will engage patients on HIV prevention topics, provide referrals, and help connect individuals to Legacy’s care continuum while serving as a member of the clinical care team.

Qualifications

  • Bachelor's degree in a related field with relevant experience preferred.
  • Experience performing point-of-care testing in clinical/community settings is desirable.
  • Good communication and customer service skills.

Responsibilities

  • Implement health education programs in one-on-one and group settings.
  • Use motivational interviewing and disease self-management techniques.
  • Provide information, referrals, and assistance to improve patient independence.
  • Conduct point-of-care testing for HIV and other conditions.
  • Contribute to new health education programs based on patient interactions.
  • Collaborate with the interdisciplinary clinic and care team.
  • Assess patient needs and readiness for health behavior change.

Health Navigator - Job Overview
Schedule: Monday-Friday (8AM-5PM)

Are you passionate about making a meaningful difference in the lives of community members? The position of Health Navigator at Legacy Community Health might be just the fit for you! In this role, you’ll build strong relationships with Legacy patients to help them access essential medical care, remain engaged in their health journey, and adhere to their medication plans for HIV prevention and treatment, all while collaborating with our dedicated Legacy Pharmacy partners.

As an essential part of our clinic and community environments, you’ll seek out individuals in need of prevention services and provide them with vital connections to Legacy’s care continuum. You’ll touch lives and empower patients on current health topics like:

  • HIV/STI testing
  • Pre-Exposure Prophylaxis (PrEP)
  • Post-Exposure Prophylaxis (nPEP)
  • HIV Treatment as Prevention (TasP)
  • Medication adherence
  • Safer sex practices
Your Responsibilities Will Include:
  • Implementing health education programming in one-on-one and group settings to enhance patients’ knowledge and wellness.
  • Using proven techniques such as motivational interviewing and disease self-management to support patients in their health goals.
  • Providing information, referrals, and assistance to improve patients’ independence and health throughout Legacy’s services.
  • Conducting point-of-care testing for HIV and other health conditions in a clinical setting.
  • Contributing to the development of new health education programs based on interactions with patients and community members.
  • Serving as a collaborative member of the interdisciplinary clinic and care team.
  • Assessing patient needs, readiness for health behavior change, and tracking progress over time.
  • Establishing and maintaining referral processes and community networks to connect patients with essential health information and services.
  • Fostering an inclusive environment by promoting culturally sensitive patient care and continuity.
  • Ensuring a high standard of customer service for a positive patient experience.
  • Documenting all patient services in the electronic health record (EHR) and other required systems.
  • Managing patient schedules in the EHR system.
  • Meeting all performance metrics, including grant and contract requirements.
  • Performing other duties as assigned to support our mission.
Minimum Qualifications:
  • High School Diploma or equivalent
  • 3 - 5 years of relevant experience
  • Bachelor's Degree in a related field with a minimum of 1 year of relevant experience preferred
  • Experience performing point-of-care testing to clients in a clinical or community setting
  • Demonstrated actions and activities with respect to HIV care and prevention
  • Provides examples of performing education to clients of diverse cultures and backgrounds in one-on-one and/or group settings.
  • Previous experience with Electronic Health Records Systems (EHRS)
  • Ability to demonstrate core competencies of HIV and HIV Prevention within 3 months of hire.
  • Excellent communication skills and customer service skills.
  • Adept in decision making responsibilities.
  • Working knowledge and understanding of statistics and graphical representation principles.
  • Strong computer skills (MS Office).
  • Demonstrated competency with an EHRS system
  • Ability to manage multiple deadlines and prioritize tasks effectively.
  • This job requires access to confidential and sensitive information, requiring ongoing discretion and secure information management
  • Ability to demonstrate core competencies of role within 90 days of hire
  • Bilingual English/Spanish fluency preferred.
