Job summary

The HUB Services Bilingual Care Coordinator, LPN is a patient-facing role in Memphis, TN, acting as the primary contact for patients, caregivers, and physicians. The LPN will educate on therapies, handle inbound/outbound calls, and document interactions to support program services and outcomes.

This position requires Spanish/English fluency, nursing experience, and flexibility to cover 7am–7pm on weekdays, with attention to accuracy and compliance across all communications.

Qualifications

• Minimum High School Diploma or equivalent.
• 2+ years nursing experience with 2+ years of other clinical work.
• Fluent in Spanish/English, strong communication skills.
• Able to work 7am–7pm, Monday–Friday shifts.

Responsibilities

• Serve as single point of contact for patients, caregivers, and physicians.
• Receive inbound calls, provide education and support on therapy areas.
• Record data and document calls to meet requirements and compliance.
• Refer adverse effects to program RN per SOPs and train as needed.
• Maintain regular attendance and participate in program meetings.
