Job summary

AltaMed Health Services is seeking a Linkage to Care Coordinator II to support the enrollment process for patients newly diagnosed with HIV or Hepatitis C, and those transferring care. You will help remove barriers to care by coordinating medications, pharmacy access, behavioral health, and case management services.

The role requires bilingual English/Spanish, a healthcare-related bachelor's degree, and prior coordination or case management experience.

Qualifications

• Bilingual English/Spanish required.
• Bachelor's degree in healthcare, social work, or a related field.
• Previous experience in healthcare coordination, case management, or social services preferred.
• A minimum requirement of a valid BLS certification or higher, following the American Heart Association (AHA) or the American Red Cross guidelines.

Responsibilities

• Facilitate seamless enrollment into medical care and ancillary services for new HIV and Hepatitis C patients and those transferring care to AltaMed.
• Help clients access medications and pharmacy services and connect to Behavioral Health and substance abuse services.
• Link clients with appropriate case management and prevention counseling resources.
