Job summary

CareSource is seeking an Intake Scheduling Specialist in Brooklyn, NY to process referrals, explain program benefits, and resolve Medicaid issues with the Entitlement department. The role schedules RN visits and ensures accurate data entry while supporting the Intake/Outreach team.

The ideal candidate has a High School diploma or GED, 2 years of healthcare experience, and proficiency with EHR/EMR systems and Outlook shared mailboxes. Bilingual English/Bengali is preferred.

Qualifications

• High School or GED required.
• Associates degree preferred.
• Two years experience in healthcare setting preferred.
• Experience with scheduling systems (EHR/EMR) and shared mailbox tools (Outlook) required.
• Bilingual - verbal and written communication required.

Responsibilities

• Process referrals by reviewing documentation for accuracy, completeness, and eligibility, and entering information into internal systems.
• Assist with obtaining follow-up documentation from providers, clients, or vendors to secure missing records.
• Schedule appointments aligning client needs with provider availability and program requirements, manage shared intake inbox.
• Coordinate with external vendors to confirm availability and smooth scheduling workflows.
• Track referral statuses and follow up to prevent delays and maintain accurate documentation.
• Support continuous workflow improvement by collaborating with the intake team.
