Job summary

AccentCare in Brooklyn seeks an Intake/Patient Care Liaison to be the primary contact for new referrals and service requests, verify insurance eligibility, and assign cases to care coordination teams. You will handle referrals and authorizations, prepare daily updates, and ensure accurate EMR documentation across departments.

Ideal candidates have a high school diploma, 1–3 years of home care experience, and bilingual English/Spanish skills.

Qualifications

• High School or GED required.
• 1–3 years of experience.
• Bi-Lingual English/Spanish required.
• Home health care or home care experience highly desirable.

Responsibilities

• Serve as the primary point of contact for new client referrals and service requests.
• Monitor intake systems and promptly assign new cases to care coordination teams.
• Verify insurance eligibility, authorizations, and client information to support uninterrupted care.
• Process referrals and authorizations within deadlines.
• Prepare daily recovery reports and communicate care updates to clients.
• Coordinate onboarding activities and confirm client demographics and service details.
• Assign caregivers to new cases and confirm schedules and needs.
• Provide excellent customer service to clients, caregivers, and partners.
