CareSource is hiring an Intake Nurse (RN) in New York. This role requires assessing new members for eligibility and explaining program details. You will work closely with the Intake/Outreach team to ensure a smooth transition for members into managed long-term care.

The ideal candidate should have an Associate's degree in Nursing, a valid RN license in New York, and at least a year of acute care experience. Flexibility for travel and evening hours may be required.

Qualifications

• Current Registered Nurse license in the state of New York required.
• One (1) year experience working in an acute care setting required.
• Home care and MLTC-intake experience preferred.

Responsabilités

• Conduct outreach in the community as needed.
• Perform the initial assessment using the UAS tool.
• Evaluate potential members for eligibility and safety.

Connaissances

Bilingual (Spanish, Russian, or Chinese)

Ability to define problems and collect data

Knowledge of Medicare and Medicaid regulations

Formation

Associates degree in Nursing

Bachelor's degree preferred
