Qualifications

Responsibilities

• Serve as primary intake contact for referrals and inquiries.
• Complete diversion assessments and follow up on case management as needed.
• Maintain referral database and provide external referrals when required.
• Represent the agency at community events and meetings.
• Follow agency Code of Conduct and Core Values.
