Summit Healthcare Staffing LLC, on behalf of a client, seeks a compassionate and detail-oriented Registered Nurse (RN) to join the Intake team in Brooklyn, NY. The role centers on coordinating patient care, collaborating with interdisciplinary teams, and ensuring patients receive a seamless start to home health services.

The ideal candidate is experienced in home health, intake, case management, or care coordination and possesses excellent clinical judgment and communication skills.

Qualificações

• Active Registered Nurse (RN) license required.
• Experience in home health, intake, case management, or care coordination preferred.
• Strong organizational and communication skills.
• Ability to multitask in a fast-paced environment.
• Experience with insurance verification and authorizations is a plus.
• Detail-oriented with excellent clinical judgment.

Responsabilidades

• Review and process incoming home health referrals.
• Verify patient eligibility, insurance coverage, and required authorizations.
• Coordinate timely admissions to home health services.
• Ensure referrals meet clinical and regulatory requirements.
• Communicate with referral sources, physicians, clinicians, and patients to facilitate a smooth admission process.
• Collaborate with the Billing and Authorization teams to ensure accurate documentation.
• Maintain compliance with agency policies and state and federal regulations.
