Responsibilities include reviewing referrals, verifying eligibility and authorizations, coordinating admissions, and ensuring regulatory compliance. Strong communication with providers and patients is essential.

Qualifications

• RN license is required and active.
• Bilingual in Spanish and English is required.
• Experience in home health, intake, case management, or care coordination is preferred.

Responsibilities

• Review and process incoming home health referrals.
• Verify patient eligibility, insurance coverage, and required authorizations.
• Coordinate timely admissions to home health services.
• Ensure referrals meet clinical and regulatory requirements.
• Communicate with referral sources, physicians, clinicians, and patients to facilitate admissions.
• Collaborate with Billing and Authorization teams to ensure accurate documentation.
