Hamaspik HomeCare in New York is seeking an experienced Intake Coordinator to help clients access services, coordinate care, and support families in our community.

The role requires bilingual Spanish and English, strong intake and care coordination skills, and the ability to manage records and liaise with clinicians and social services. You will work closely with families and care teams to ensure compassionate, compliant service delivery.

Qualifications

• Fluency in both Spanish and English is required.
• Experience in intake, care coordination, or case management is preferred.
• Strong organization and attention to detail.

Responsibilities

• Serve as the primary contact for prospective clients and families, sharing information about Hamaspik’s services and procedures.
• Conduct intake assessments to identify needs and determine appropriate services.
• Gather medical history, medications, allergies, and insurance details.
• Coordinate individualized care plans with clients, families, and nursing staff.
• Assist current clients with applications for additional services for which they may be eligible.
• Refer clients to social service agencies and community resources as needed.
• Maintain accurate client records and documentation.
• Build relationships with clients, families, and community partners through effective communication and follow-up.
• Ensure confidentiality and compliance with regulations and policies.
• Participate in team meetings and training to stay current.
