Job summary

Bond Health in Brooklyn seeks a Case Manager to oversee a 105-unit Permanent Supportive Housing program. The Case Manager will provide case management and service coordination at the program site, with frequent contact through face-to-face meetings, calls, e-mail, and other electronic exchanges with colleagues.

The role includes assessments, person-centered service plan development, referrals to benefits, collaboration with the psychiatrist, and community integration using Housing First

Qualifications

• Bachelor's degree in human services or related field.

• Experience with homelessness, substance use, mental health and/or trauma settings preferred.

• Bilingual English/Spanish required.

• Weekend availability.

• Must possess strong organizational, writing, communication, and computer skills.

• Self-starters with energy to work with high-need individuals.

• Strong engagement and interpersonal skills.

• Some experience with low-income or homeless populations.

• Familiar with interventions for residents with severe persistent mental health issues.

Responsibilities

• Maintain a caseload of high risk/high intensity tenants.
• Provide enhanced case management services informed by Housing First in the office and at residents' homes.
• Conduct assessments and program intakes for assigned residents.
• Ensure data accuracy and collect required documentation.
• Develop and update person-centered service plans (every 6 months).
• Utilize trauma-informed interventions and culturally responsive engagement techniques.
• Identify residents’ strengths, needs, abilities, and preferences.
• Act as liaison for the psychiatrist and manage appointment times.
• Provide referrals, advocacy, and linkages to public benefits.
• Maintain and refer to a network of resources to support community integration.
• Implement groups focusing on life skills, education, support, relapse prevention and mental health counseling.
• Coordinate with Workforce Opportunities to link residents to employment/education assistance.
• Coordinate aftercare services and follow-ups post-discharge.
• Conduct outreach and engagement including apartment visits/inspections.
• Participate in community development and partnerships with other providers.
• Ensure compliance with regulations and complete monthly Audit Tool.
• Work with Property Management to ensure lease terms and rent payments.
• Provide services respectfully across cultural differences.
• Provide administrative support to leadership as needed.
• Other duties as assigned.
