Total Care Connections (TCC) seeks a bilingual Home Visit Coordinator (Spanish & English) to support clients in Tempe, AZ. You’ll conduct in-home assessments, guide admissions, and coordinate care transitions with empathy and professionalism.

You’ll visit clients, document needs, and communicate with scheduling and care teams while maintaining thorough records. Fluency in Spanish and English, strong communication, and discretion are essential in this client-facing role.

Qualifications

• Experience in home care, healthcare, caregiving, senior care, or client-facing service environment preferred.
• Strong communication skills, both verbal and written.
• Ability to manage multiple priorities and maintain confidentiality.

Responsibilities

• Build trusting relationships with clients, families, and referral partners.
• Conduct in-home visits and client assessments.
• Coordinate onboarding and care transitions with Scheduling and Care teams.
• Document assessments, visits, and care plans thoroughly.
• Participate in weekend on-call responsibilities as needed.
