Job summary

Family and Friends Homecare in Lakewood, CO is seeking a Homecare Care Manager who is bilingual in English and Spanish. You will be the primary contact for clients, families, caregivers, and referral sources, ensuring smooth service delivery and clear communication.

The role involves client intake, creating HCBS/IHSS Medicaid care plans, coordinating with case managers, understanding Medicaid PARs, and conducting quarterly supervisory visits, while maintaining accurate records in a fast-paced

Qualifications

• Bilingual fluency in English and Spanish is required.
• Experience in care coordination or home care is preferred.
• Excellent organizational and communication skills.
• Comfort with Medicaid knowledge is helpful.

Responsibilities

• Serve as a primary point of contact for clients, families, caregivers, and referral partners.
• Meet with potential clients and their families.
• Perform intake visit with New Clients.
• Create Care Plans for HCBS and IHSS Medicaid Clients.
• Communicate with Case Managers regarding Client Care Plans.
• Understand Medicaid Eligibility and PARs.
• Schedule and Complete Quarterly Supervisory Visits with Clients.
• Assist with the Incident/Complaint Reporting process, following up on incidents or complaints and ensuring proper and timely communication.
• Maintain accurate client records.
