Job summary

CSI, a Help at Home Company, based in Tampa, FL, is seeking a Care Coordinator to support clients and caregivers. You will schedule home care aides, coordinate care plans, and maintain accurate records to ensure a top-tier client experience in home settings.

The ideal candidate is bilingual (English/Spanish), highly organized, and experienced in health care or home health, with strong HIPAA and documentation skills. Some local travel may be required.

Qualifications

• Fluent in both English and Spanish.
• Experience in health care, preferably in home health.
• Staffing/scheduling experience preferred.
• Medicaid, Waiver, or Home Healthcare experience preferred.
• Other state or local rules may apply.

Responsibilities

• Maintain client and employee files and related paperwork.
• Ensure HIPAA compliant handling of patient records.
• Assign homecare aides based on client needs and Plan of Care.
• Monitor performance of homecare aides for quality service.
• Communicate schedules and changes with clients, aides, referrals, and managers.
• Conduct in-home client visits to maintain relationships.
• Prepare and submit routine departmental reports.
• Record and maintain documentation of client conditions and services.
• Handle client complaints and escalate issues as needed.
• Provide education on changes to a client’s Plan of Care.
