Addus HomeCare in Oakland, CA is seeking a Service Coordinator to schedule and supervise in-home care workers, conduct visits, and ensure high-quality service for older adults and individuals with disabilities. The role emphasizes problem-solving, coordination, and compassionate client support.

The position is full-time, Monday–Friday, 8am–5pm, offering benefits and opportunities to impact clients’ daily living and wellbeing.

Qualifications

• Must have high school diploma or equivalent.
• 3 to 5 years of Industry experience required.
• Interpersonal, organizational and communication skills.
• Computer skills including but not limited to Microsoft Word, Microsoft Excel and Scheduling program.

Responsibilities

• Schedule and supervise in-home care workers and clients within a geographic area.
• Enter and adjust schedules, respond to client requests, and ensure care plans are followed.
• Conduct monthly client wellness calls and home visits as required.
• Provide escalation follow-through on client complaints and theft claims.
• Screen and support field staff; enforce attendance and conduct policies.
