Qualifications

Responsabilités

• Coordinate and oversee homecare services per care plans.
• Monitor aide performance and service delivery.
• Act as liaison between participants, families, and IDT.
• Ensure timely documentation and authorization updates.
• Support onboarding and coaching of aides and vendor staff.
