BoldAge PACE in Fresno, CA is seeking a Licensed Practical Nurse (LPN) who is bilingual in English and Hmong. Under supervision, you will administer medications, implement care orders, and communicate with the interdisciplinary team to ensure quality patient care.

Qualifications

• Bilingual fluency in English and Hmong.
• Graduate of an accredited school of nursing.
• Current LPN/LVN license in good standing.
• BLS required (within 90 days of employment).
• IV Certification preferred.

Responsabilités

• Administers medications and implements primary care orders.
• Documents nursing care in participant records and reports.
• Coordinates with inter-disciplinary team and RN Care Manager.
• Educates participants and caregivers about medications and care plans.
• Maintains clinic equipment, supplies, and maintains safety standards.
• Schedules and assists in appointments and admissions as requested.
