Job summary

El Programa Hispano Católico (EPHC) seeks a Health-Related Social Needs (HRSN) Community Health Worker in Oregon to connect OHP members with housing supports, community resources, and social services. This frontline role emphasizes trauma-informed, person-centered care and Housing First principles to promote stability, health, and wellbeing.

The CHW builds trusted relationships among participants, healthcare providers, and social service agencies, guiding them through housing barriers and access

Qualifications

• Associate’s degree in public service, social work, education, healthcare, or human development.
• Minimum of 1 year of experience providing case management, information & referral services to individuals and families.
• Bilingual in Spanish and English, with bicultural competency.
• Ability to manage multiple cases effectively and work in fast-paced environment.

Responsibilities

• Conduct outreach and education to promote HRSN services.
• Screen for HRSN eligibility and respond within expected timeframe.
• Develop individualized case management plans and provide referrals.
• Maintain confidential participant records and documentation.
