Job summary

Neighborhood House in Portland, OR is hiring a Bilingual Community Based Care Coordinator to deliver Health-Related Social Needs (HRSN) care coordination, outreach, and tenancy supports to clients. You will manage a substantial caseload and document progress in our client management system, ensuring timely, culturally responsive services.

This full-time role emphasizes trauma-informed care, strong collaboration with NH programs and external partners, and a commitment to equity in service

Qualifications

• Minimum 3 years of experience in community health work or related field.
• Bilingual in English and Spanish or Ukrainian.
• Proficient with Microsoft Office and client documentation systems.

Responsibilities

• Deliver health-related social needs care coordination and outreach.
• Hold a caseload of 28-30 clients with timely documentation.
• Provide tenancy support and link clients to housing resources.
• Develop individualized care plans with client input and goals.
• Ensure trauma-informed, culturally responsive services.
• Coordinate with internal and external partners for referrals.
