Job summary

Suvida Healthcare seeks a Senior Guia to provide clinical social work and complex case management for high-risk patients and families. You’ll assess plans of care, coordinate resources, and ensure care transitions across in-home, clinic, and facility settings.

Qualifications

• Graduate of an accredited MSW program preferred.
• Licensed Master Social Worker (LMSW) or equivalent with 3–5 years of experience and CHW certification.
• Knowledge of government/community resources and Medicare/Medicaid.
• Knowledge of LIS, PAP, and SNAP applications.
• Strong critical thinking, problem-solving, communication, and time management skills.

Responsibilities

• Provides comprehensive care coordination to an assigned patient caseload.
• Works collaboratively with patients, family, caregivers, healthcare providers, and external partners.
• Develops and carries out treatment plans using a clinical social work diagnosis and interventions.
• Intervenes with patients and families regarding emotional, social, and financial consequences of illness.
• Assesses and mobilizes family/community resources to meet social care needs.
• Supports transitions of care planning and referrals to post-acute providers.
