Suvida Healthcare in Arizona is seeking a bilingual (English/Spanish) Physical Therapist with 3+ years of PT experience to join our integrated primary care team serving Medicare-eligible seniors. You will provide patient-centered therapy, develop movement programs, and collaborate with a multidisciplinary team to improve function, manage pain, and enhance quality of life.

The role offers a $1,500 sign-on bonus and a manageable caseload, with opportunities to lead fall prevention programs,

Formación

• DPT degree from an accredited physical therapy program.
• Active Arizona PT license or ability to obtain one.
• Current BLS certification.
• 3+ years PT experience, preferably with geriatrics.

Responsabilidades

• Perform thorough PT evaluations using clinical observation and standardized tests.
• Develop individualized, evidence-based treatment plans to improve function.
• Provide patient- and caregiver-centered education to enhance movement and activity.
• Lead fall prevention classes and complete fall risk screenings.
• Collaborate with interdisciplinary teams to identify goals and optimize care.
• Supervise and mentor PTAs and interns.
