The Transitional Foster Care Case Manager at Catholic Charities West Michigan coordinates case management and release planning for youth in transitional foster care, ensuring timely assessments, service coordination, and reunification efforts with families and sponsors.

Qualifications

• Bachelor's degree in behavioral sciences, human services or social services fields is required or preferred.
• Minimum 1 year of experience in child and family services or counseling is encouraged.
• Bilingual Spanish/English is highly preferred.

Responsibilities

• Coordinates intake assessments within 24 hours of arrival and orients youth to the program.
• Maintains regular contact with youth and provides weekly updates on reunification progress.
• Facilitates sponsor engagement and coordinates release planning with family and partners.
• Documents all contacts, assessments, and case reviews in required data systems.
• Develops and updates individualized case plans and monitors progress with the team.
• Attends required meetings and ensures services are provided on-site with confidentiality.
