Job summary

ForzaCare is seeking a Field Medical Case Manager to coordinate care for injured workers, connecting providers, employers, and insurers to enable timely recovery and return-to-work.

Ideal candidates are bilingual in English and Spanish, hold an RN or CRC license, and have workers' compensation experience. The role may involve travel to appointments and sites to ensure comprehensive care.

Qualifications

• Bilingual in English and Spanish is required.
• Active RN or CRC license with associated degree is required.
• CCM, CIRS or other case management credentials are preferred.
• Must travel to appointments with valid driver's license and transportation.
• Experience in workers' compensation case management is strongly preferred.

Responsibilities

• Coordinate care among medical providers, employers, insurers, and injured workers.
• Attend appointments and travel to worksites as needed.
• Develop, document, and monitor recovery goals and return-to-work plans.
• Provide consistent progress reports to clients and stakeholders.
• Ensure compliance and maintain strong relationships with clients and team members.
