Job summary

Pair Team is expanding its partnership with one of our largest health plan partners through an exciting new field-based care management role focused on improving patient engagement and access to care throughout Alameda County.

The Field Care Coordinator will be in the community five days per week, handling 120 visits per month, traveling 30–40 miles daily, onboarding new patients, delivering care packages, and coordinating services across primary care, behavioral health, and social resources.

Qualifications

  • Bilingual English/Spanish is required.
  • 1+ years of field-based (in person) case management or care coordination.
  • Physical location in or near Alameda County with familiarity of the local community and resources.
  • Reliable transportation for regular field visits to patient homes, clinics, community-based organizations, and partner sites.
  • Valid driver's license and current auto liability insurance.
  • Strong understanding of cultural fluency and ability to build trust with diverse populations.
  • High degree of empathy and ability to remain patient when faced with adversity.
  • Demonstrated ability to work collaboratively in a multidisciplinary care team.
  • Excellent organizational skills and attention to detail.
  • Strong technical skills and comfort learning new systems; experience with Google Suite, Slack, CRM databases, Excel, Word, email, and video conferencing.

Responsibilities

  • Manage a high-volume, field-based caseload with approximately 120 visits per month.
  • Independently manage a busy daily schedule while traveling throughout Alameda County.
  • Support recurring community-based events and provide care coordination for patients with complex needs.
  • Serve as the primary contact for members with complex medical, behavioral and social needs.
  • Meet members in person at homes, in the community, or at partner organizations for onboarding or delivering care packages.
  • Conduct in-person visits to assess member needs and encourage adherence to care plans.
  • Coordinate services, appointments, and referrals across primary care, behavioral health, and social resources.
  • Collaborate with Nurses and Behavioral Health Care Managers to maintain care continuity.
  • Identify and address barriers to care such as transportation, medication access, or health literacy.
  • Develop and maintain individualized care plans and ensure follow-up on all interventions and goals.
  • Maintain accurate documentation of all member interactions and care coordination activities.
  • Advocate for member needs within healthcare and social support systems.
  • Participate in multidisciplinary care meetings to review patient panels and align on care plans.
