Job summary

Pair Team is expanding its field-based care management role in Alameda County. The Field Care Coordinator will provide hands-on support, onboarding new patients, and helping overcome barriers to access, delivering care directly in the community.

The role requires 1+ years in field-based case management, strong organizational skills, and fluency in English and Spanish. Travel within Alameda County is expected, with daily field visits and community outreach.

Qualifications

• 1+ years of field-based (in person) case management or care coordination.
• Bilingual English/Spanish proficiency required.
• Must have reliable transportation and valid driver's license.

Responsibilities

• Manage a high-volume field-based caseload with in-person visits across Alameda County.
• Meet members where they are, in homes or community sites, five days a week.
• Coordinate services across primary care, behavioral health, and social supports.
• Document interactions accurately and maintain care plans with follow-up actions.
• Collaborate with Nurses and Behavioral Health Care Managers on patient panels.
