Primary Health Care, Inc. is seeking a Family Support Worker in Des Moines, IA. You will assist patients by identifying needs, connecting them to resources and applying for entitlements, while delivering compassionate care in a diverse population.

Verbal fluency in Spanish and English is required. The role includes outreach, documentation, translation support, and collaboration with Patient Care Teams to enhance service experiences.

Formación

• Must be bilingual in English and Spanish (verbal and written).
• High school diploma or equivalent required.
• Knowledge of community resources and entitlement programs preferred.
• Strong verbal and written communication skills; empathetic, patient approach.

Responsabilidades

• Identify patient needs and connect them to community resources and entitlement programs.
• Assist uninsured individuals and families with accessing mainstream resources and completing applications.
• Conduct outreach in community settings and with PHC clinics to support clients.
• Document client contacts per PHC policies and participate in trainings and meetings.
• Provide interpreting/translation support and assist with front desk operations as needed.
• Perform other duties as assigned.
