Formación

• Minimum 1 year experience in human services, healthcare, or related field.
• Knowledge of community resources, including entitlement programs.
• Reliable transportation to perform outreach activities.

Responsabilidades

• Assist uninsured individuals and families in identifying needs.
• Conduct outreach activities to uninsured individuals.
• Document all client contacts in compliance with PHC policies.
