Qualifications

Responsibilities

• Facilitate enrollment into the program with caregivers and child.
• Provide monthly home visits to address family needs, parenting skills, and behavior planning.
• Coach caregivers on responsive parenting and child development.
• Maintain ongoing communication with families to monitor progress and provide support.
• Collaborate with transportation and classroom staff to coordinate services.
• Maintain a caseload of 15–20 families and conduct MDT meetings every 90 days.
• Document all visits and meetings in the electronic record per policy.
