Responsibilities cover enrollment, family coaching, and coordination of MDT meetings, with detailed documentation and strict confidentiality.

Qualifications

• Bachelor’s degree in education, human development, family support, social work, psychology, or related field.
• Two years’ experience with caregivers and at‑risk children preferred.
• Bilingual in English and Spanish required.
• Strong behavioral management and communication skills.

Responsibilities

• Provide intensive case management with monthly home visits.
• Recruit and enroll, complete enrollment paperwork.
• Coordinate MDT meetings every 90 days to review care plans.
• Maintain documentation within 48 hours; HIPAA/FERPA compliance.
