Job summary

San Andreas Regional Center is seeking a qualified professional to provide and coordinate services to consumers under the Lanterman Act. The role requires strong data gathering, assessment, and planning within an interdisciplinary team, with emphasis on person-centered planning and advocacy.

You will work to ensure timely access to services, maintain IPP documentation, and help families navigate eligibility and funding processes while upholding agency policies and quality standards.

Qualifications

• Bachelor's degree in social work or related field.
• Able to perform assessments and develop IPPs in a timely manner.
• Experience collaborating with interdisciplinary teams and advocating for clients.

Responsibilities

• Gathers data and produces initial and ongoing assessments.
• Presents assessment data to IDT for eligibility decisions.
• Coordinates the Person-Centered Planning process with consumers and families.
• Counsels consumers on identifying and achieving personal goals.
• Advocates to ensure access to needed services for consumers.
• Initiates and follows up on referrals to community resources.
• Maintains required caseload documentation, including IPP.
• Explains provisional eligibility clearly and concisely.
• Performs related tasks as needed.
