Qualifications

• Bachelor’s Degree in social work, rehabilitation counseling or related field.
• Valid driver's license and automobile liability insurance.
• Bachelor's in Social Work plus four years of experience may be required for specific roles.

Responsibilities

• Gathers data and performs initial/ongoing assessments.
• Presents data to IDT for eligibility.
• Coordinates Person-Centered Planning with consumers and families.
• Counsels consumers on personal goal achievement.
• Advocates to ensure provision of needed services.
• Initiates and follows up on referrals to community resources.
• Documents caseload including IPP.
