Job summary

San Andreas Regional Center is actively seeking a dedicated social work professional to provide and coordinate services for consumers under the Lanterman Act. This role emphasizes advocacy, planning, and collaboration with families and the interdisciplinary team to support independence and inclusion.

Responsibilities include data gathering, assessment, planning, and ensuring access to needed services. The position includes a monthly stipend for bilingual speaking and a comprehensive benefits

Qualifications

• Knowledge of principles, objectives and techniques of social work; human growth, family relationships, and community resources.
• Ability to relate constructively to clients, families, and colleagues; prepare reports and maintain case records clearly.

Responsibilities

• Gathers data and produces initial and ongoing assessments.
• Presents assessment data to Interdisciplinary Team for eligibility.
• Coordinates Person-Centered Planning with consumers and families.
• Counsels consumers on setting and achieving personal goals.
• Advocates to ensure provision of needed services.
• Initiates referrals to community resources and tracks timelines.
